Family Practice Physician to $230,000 + Bonus+ Benefits +++++

 

We are seeking a number of Family Practice Physicians for our clinics throughout Indiana. Our clinics primarily serve the underserved, uninsured, Medicare and Medicaid patients along with the general population.

We are seeking Physicians that desires to service underserved communities and provide great healthcare to those that need it most. You have a passion for healing!

  • East Chicago, Indiana
  • Knox, Indiana
  • Michigan City, Indiana
  • Mishawaka, Indiana
  • LaPorte County, Indiana
  • South Bend, Indiana

 

We offer the following:

  • Starting Base Salary of $200,000 to $230,000 (Flexible depending on experience)
  • Annual Bonus of 3-5% of annual salary
  • 4 weeks Paid Time off first year / Vacation

As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients. This position will work closely with the clinical staff and will report to the Site Medical Director.

JOB RESPONSIBILITIES:

  • Provide medical care to all members of a family including children and seniors.
  • Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.
  • Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and “no show” appointments.
  • Provides whole-person care which includes appropriate
    • referrals to the social worker, nutritionist, and mental health professionals.
    • integration of biologic, social, psychological and environmental issues.
  • Maintains adequate, concise, and legible documentation of patient care.
  • Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)
  • Educate patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.
  • Utilizes appropriate infection control measures, including universal precautions.
  • Supervises advance practice nurses in Clinic when assigned.
  • Provides consultation and prescription signatures to advance practice nurses when needed.
  • Oversees residents in the management of hospital patients.
  • Writes daily progress notes in hospital charts.
  • Takes phone calls about hospital patients.
  • Participate in assigned Quality Assurance/Management in Daily Work activities.
  • Regularly attends QA/TQM as required.
  • Participates in the physician performance audit as assigned.
  • Assists in the development and maintenance of Clinic practice guidelines (protocols) as assigned.
  • Participates in Total Quality Management (TQM) activities. 
  • Attends Clinic’s Provider Group Meetings and hospital section meeting as scheduled.
  • Attends required in-service meetings.
  • Makes presentations in the community when requested.
  • Teaches students and residents as assigned.
  • All staff are committed to engage in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH).
  • Performs other duties as assigned.

QUALIFICATIONS:

Education/Training

  • Graduation from an approved medical school
  • Completed Residency Program
  • Board certified or board eligible
  • DEA certification
  • CPR certification
  • Indiana State Medical License or who can obtain a State Medical License
  • Continuing medical education

Work/Life Balance

It is a 40-hour work week with 32 hours in patient care and 8 hours in admin duties.
